Answer: Herpes virus infection
Histology: The skin is extensively ulcerated and inflamed (image 1). Scattered throughout the epithelium are numerous multi-nucleated cells with classic nuclear changes of a Herpes virus (HSV) infection (image 2). An immunostain for HSV was strongly positive (not shown).
Discussion: This dramatic pathology was produced by a Herpes virus infection. HSV infection of the scrotum often occurs in immunocompromised patients.
