Answer: Metastatic renal cell carcinoma to scar
Histology: none provided
Discussion: The lesion has clear cell features and is positive for PAX8 and CD10 and negative for p63, consistent with a metastatic renal cell carcinoma to the scar of previous surgery. It is a diagnosis to keep in mind in case of a clear cell tumor, especially that renal cell carcinoma can metastasize to various sites including other tumors.
