Answer: Mullerianosis
Histology: see below
Discussion: Mullerianosis of the urinary bladder is an extremely rare encounter. The tumor-like lesion may contain one or more of several types of benign Mullerian -type epithelia including endometrial, endocervical, and endosalpingial tissue. Possible mechanisms of pathogenesis include implantation or metaplastic origins, the latter has been recently favored. The lesion has a benign biologic behavior and may be associated with other metaplastic changes such as cystitis cystica glandularis.
A case of endometrioid carcinoma and very rare cases of clear cell adenocarcinoma originating in endometriosis or mullarian cysts of bladder are on record
