Answer: Acinar cell carcinoma
Histology: This neoplasm arose in the salivary gland and was composed of sheets of large polygonal cells with basophilic granular cytoplasm.
Discussion: Acinic cell adenocarcinomas of the salivary gland are malignant epithelial neoplasms which, as the name suggests, show differentiation toward serous acinar cells. Acinic cell adenocarcinomas account for about 20% of all malignant salivary gland tumors. Most arise in the parotid gland. Grossly they can be well-circumscribed, and microscopically, as in this case, they typically are composed of sheets of large polygonal cells with granular basophilic cytoplasm. These neoplasms will immunolabel with antibodies to amylase and variably for cytokeratins.
