Answer: Metastatic colloid carcinoma
Histology: This lymph node has been replaced by pools of relatively acellular colloid.
Discussion: This patient had a history of a colloid carcinoma of the pancreas. As is true for most colloid carcinomas of the pancreas, this patient’s carcinoma associated with an intraductal papillary mucinous neoplasm of the pancreas. The tumor in the lymph node shows extensive treatment effect. Immunolabeling for cytokeratin, however, revealed scattered viable epithelial cells floating in the pools of extracellular mucin helping to establish the diagnosis.
