This 60 year old with a remote history of a cholecystectomy developed a firm nodule in his rectus abdominus muscle. A CT scan revealed that the mass was calcified. The lesion was resected.images/RHH1 01.jpg

Answer: Ectopic glands with reactive bone formation
Histology: This well-demarcated nodule is composed of mature bone and soft tissue with rare admixed glands. The glands are lined by uniform cells without nuclear atypia.
Discussion: Although the glands are in an abnormal location, they lack atypia. A review of the patient’s history revealed a remote history of a complicated cholecystectomy in which there was reported to be disruption of the gallbladder. Non-neoplastic epithelial cells must have shed from the gallbladder and implanted in the rectus abdominus muscle.
