Answer: Schwannoma
Histology: This spindle cell neoplasm also abuts the pancreas. Unlike the case presented above (case 2), this tumor has a lymphoid cuff at the periphery of the tumor. An immunostain for S-100 protein is strongly positive.
Discussion: Schwannomas, as is true in fact for almost all mesenchymal neoplasms, may occasionally abut or even involve the pancreas. In this case, the lymphoid cuff at the periphery of the tumor suggests the diagnosis of a schwannoma, and the diagnosis is supported by the characteristic nuclei and immunolabeling for S-100 protein.
