Answer: Gastrointestinal stromal tumor, high risk
Histology:
Discussion: This is a malignant spindle cell lesion with extensive necrosis and a broad differential diagnosis including melanoma, sarcomatoid carcinoma, sarcoma (dedifferentiated liposarcoma, MPNST, leiomyosarcoma), malignant GIST (history), spindle cell mesothelioma. The patient has a previous history of GIST and the tumor is c-kit positive. The AJCC 7th edition has introduced a staging for GIST. The parameters to be considered in the staging are: size: <2cm; 2-5cm; 5-10cm ;> 10cm, mitotic rate: low= <5/50HPF and high=>5/50HPF, lymph node metastases, and distant metastases. The stages are different for gastric and for small intestinal tumors.
