Discussion: The lesion of diabetic mastopathy consists of a densely collagenous stroma and an increased concentration of stromal spindle cells when compared to the surrounding breast tissue. Epithelioid cells are found dispersed in the collagen among spindle cells. Mature lymphocytes are clustered circumferentially around small blood vessels and breast lobules and ducts throughout the lesion. Although these changes mostly occur in patients with insulin-dependent diabetes mellitus they are not specific for diabetes and can occur in other auto-immune diseases and is typically an abnormality occurring in premenopausal women. In cases of diabetic mastopathy, the patients usually present with a unilateral or bilateral firm to hard tumor. The disease in the breast is usually self limited.
