Answer: Meningioma metastatic to the pleura
Histology: This spindle cell neoplasm is remarkable for a syncytical growth pattern and focal “whorls.”
Discussion: The vast majority of meningiomas are entirely benign. Only 0.1% metastasize. They tend to be EMA positive. Other entities to consider in this case include the solitary fibrous tumor (SFT). SFTs usually express CD34 and bc12.
