Answer: Mucinous tubular and spindle carcinoma
Histology: Foci of this tumor has the typical morphology of Mucinous Tubular and Spindle Carcinoma (so-called Henle Loopoma); that is, the tumor is relatively circumscribed, features interconnecting tubular and spindled architecture within a myxoid stroma, and consists of cells with low grade nuclei and eosinophilic cytoplasm. In one area of this tumor, the tubules are prominently collapsed, giving rise to a spindle cell morphology that simulates a sarcoma or sarcomatoid carcinoma. These areas are diffusely immunoreactive for cytokeratin.
Discussion: A sarcomatoid carcinoma would show greater pleomorphism than is seen in the current case. Papillary renal cell carcinoma would typically show more extensive true papillae, and stromal foam cells. Similarly, a high grade collecting duct carcinoma would show greater cytologic atypia, acutely inflamed stroma, and more prominent desmoplasia than the current lesion.
