Answer: Chronic pancreatitis
Histology: The pancreas is fibrotic and there is significant loss of acinar tissue. A collection of neuroendocrine cells adjacent to a nerve is seen.
Discussion: As discussed in case #1, very rarely one can see a benign gland adjacent to a nerve in the pancreas. This is also true for endocrine cells. In rare cases of chronic pancreatitis one can see collections of endocrine cells (islets of Langerhans) adjacent to nerves. This feature should therefore not be used as the sole criterion for distinguishing between a reactive process and an endocrine neoplasm.
