Histology: The tumor is composed of multiple cysts lined by flat or cuboidal cells with eosinophilic cytoplasm. The nuclei of tumor cells display prominent nucleoli and the cells have a characteristic hobnailing of the nuclei into the luminal spaces of the cysts. The stroma is hyalinized.
Discussion: Tubulocystic carcinoma is a rare subtype of renal cell carcinoma that is characterized by its sponge-like cut surface on gross examination that it is given by the tubular and cystic structures that the neoplastic cells forms. The cells have scant and eosinophilic cytoplasm and, therefore, one of the most frequent diagnostic problems is with oncocytoma with tubular pattern. In contrast to oncocytoma, the stroma of tubulocystic carcinoma is hyalinized and not edematous or myxoid as in oncocytomas. Tumor cells have prominent nucleoli but nuclear grade is not applicable in these tumors as the great majority of them have and excellent prognosis with no recurrence or metastasis.
