Answer: Duodenal diverticulum
Histology: The cystic lesion is composed of mature intestinal-type epithelium. This epithelium has retained the architecture of the normal duodenal mucosa.
Discussion: Duodenal diverticuli sometimes extend into the pancreas and when they do they can mimic an intraductal papillary mucinous neoplasm. Clues to the diagnosis include the demonstration of an orifice in the duodenum leading into the cystic lesion, an absence of atypia and retention of the architecture of the normal duodenal mucosa.
