Answer: Alveolar soft part sarcoma
Histology: This is a nested proliferation of eosinophilic polygonal cells which are discohesive, imparting an alveolar architecture. The neoplastic cells have vesicular chromatin and prominent nucleoli, and demonstrate significant cytologic atypia. By immunohistochemistry, the neoplastic cells are negative for cytokeratin and S100 protein, but demonstrate diffuse nuclear immunoreactivity for TFE3 protein.
The presence of a TFE3 gene fusion was confirmed by fluorescence in situ hybridization.
