Answer: Angiomyolipoma (AML)

Discussion: AML is the most common mesenchymal tumor in the kidney. It is part of the PEComas group of tumors. AML can occur in the setting of tuberous sclerosis (small, bilateral and multifocal). In adults multifocality and lymph node involvement can occur and are not considered as metastases. Unlike classical AML (composed of smooth muscle spindle cells, adipose cells, and vessels in variable proportions), epithelioid AML especially when associated with atypical features can have a malignant behavior and it is advised that these tumors be closely followed.
