Answer: Metanephric adenoma

Discussion: Metanephric adenoma is a benign well circumscribed renal neoplasm. The main differential diagnosis includes the solid variant of papillary renal cell carcinoma and epithelial-predominant Wilms tumor. In contrast with metanephric adenoma, papillary renal cell carcinoma labels diffusely with CK7 and EMA; it is WT-1 negative. Wilms tumor and metanephric adenoma are both positive for WT-1 indicating a relation between these 2 tumors. However Wilms tumor displays cytologic atypia, abundant mitotic activity and occurs at a younger age.
