Discussion: The background liver shows cirrhosis secondary to steatohepatitis. Hepatocellular carcinoma can arise in the setting of non-alcoholic steatohepatitis (NASH) with cirrhosis. Some studies have found that the five-year cumulative incidence of hepatocellular carcinoma in the setting of NASH can be as high as 7%.
