Answer: Adenoid cystic carcinoma
Histology: The lesion consists of malignant cells arranged mainly in a cribriform pattern, but also tubular patterns a focal solid areas are seen. The cells are mildly atypical with hyperchromatic nuclei and high N/C ratio arranged around gland-like spaces. The lumens contain basal lamina material or mucin. Extensive perineural invasion is seen.
Discussion: The histological appearance of the tumor is typical of adenoid cystic carcinoma. Primary adenoid cystic carcinoma of the mandible is rare. They are similar histologically to the tumors arising in the salivary glands. It is actually cold be arising from ectopic salivary gland tissue that can be rarely found in the mandible among other head and neck locations, or from mucus secreting cells found in the epithelial lining of dentigerous cysts.
